MIDDLE SCHOOL 

Vaccination Requirement:

7th-grade students are required to have the Meningitis and Tdap vaccine. Documentation must be provided to BWMS within the first 2 weeks of the start of the school year. Physicians can fax documentation to 740.965.6471, Attn: Nurse

7th Grade Students who do not have the required immunizations within the first 2 weeks of school will be excluded from school until they meet immunization requirements.

Vaccination Requirement:

12th-grade students who do not have the required immunizations within the first 2 weeks of school will be excluded from school until they meet immunization requirements.

Ohio law mandates that students be fully immunized against vaccine-preventable diseases. While vaccinations are required, Ohio law also allows for exemptions for medical reasons, as well as for reasons of conscience, including religious convictions. Students who have an exemption must provide documentation to the school, either in the form of an exemption form or a medical statement from a physician. Proof of vaccination or exemption forms are required within 14 days of the start of student enrollment; otherwise, students will be exempted until required documentation is provided.
